These are the steps to get started on your journey with conquest centers
1. Fill Contact Form
First, we ask that you fill out our contact form in the bottom right corner of this page. A member of our team will then contact you and set up a phone interview time.
2. Phone interview
One of our team members will call and gather all the necessary information to enroll your child over the phone, and then we will send you the intake form and all other necessary paperwork
3. Verify Insurance
Based on the intake form and other paperwork submitted by you, we can begin verifying insurance benefits
4. Schedule a Tour
We are confident our centers are the best setting for ABA therapy, but don’t take our word for it – schedule a tour of our center, meet our team, and see for yourself!
5. Child evaluation meeting
Before treatment begins, your child will be scheduled for an evaluation meeting (which can take 2-3 hours).
6. Prepare treatment plan
Following the assessment, we will develop a unique treatment plan with your child’s specific goals in mind. A team of RBT’s will be assigned and will welcome you and your child to conquest centers.
